The architecture I use is Microsoft Geo-Cluster which runs across dark fiber to a secondary site about 25 miles from the primary. The hardware is all server class equipment with hot swappable and redundant components. The Geo-cluster uses redundant EMC DMX SANs using SDRF-CE to provide simultaneous data replication. If the cluster fails over then storage and server fail as well.
All applications use DNS to point to the servers - also is redundant. Applications are mandated to be cluster aware and account for failover.
Databases are backed up to a virtual tape library offsite. Backups are tested both full restore and log restore once a week. The operating system and other software are patched in a QA environment then in production within 30 days of patch release. All software required to be at a current support level from the vendor.
SQL is configured for the minimum services and a conservative approach using tuning measures that will foster stability.
SQL servers are on a separate network, firewalled and monitored for security threats, denial of service threats etc. There are some other nuances but in a nutshell that it.
